5 weeks is too young to be given a puppy, they leave their moms at 8 weeks. Take the dog back to finish being weaned. Whoever has the mother dog is not doing good by giving away puppies this young.

Sure. Why did you take it before it was weaned?

Anyway, bottle-feed your 5-week old puppy with Similac or something similar and get it started on soft puppy food as soon as it can tolerate it.
Get it to the vet also and get its puppy shots right NOW.

At 5 weeks old your puppy should still be with mom but beginning to be weaned on puppy chow moistened

If your puppy can not be with mom, you can get puppy milk at a pet store. See if the puppy will lap it like water. You can also start introducing puppy food soon. You may mix the food with puppy milk in the beginning. Talk to a vet and to the people at the pet store about when and how. Also, read the label for amounts based on the puppy's size.

it's not been weaned.

Most places won't allow you to get any of their puppies (if good breeders) unless the puppy is at least 6 weeks!

If you can't give it it's mom, then go get a good quality puppy chow and some puppy milk formula. Mix up the formula then put a small handful of the puppy chow in the bowl, pour puppy milk on top. Let sit for 5 minutes.

Give it to the puppy. Most 5-weeks-old puppies have no idea how to really lap from a bowl - you have to show them gently. Dip your fingers in the milk (which should be lukewarm.) then offer your finger to the puppy. Try to get your finger very close to the rim of the bowl. (You may have to redip your finger but it's fine!) Now with your finger smear some of the milk on the rim. The puppy will start lapping it and try to get more. A lot of times ending up with milk on their noses.

The puppy then will lap the milk up and it will learn. BOWL = MILK. when it gets older 7-8 weeks (when you're actually SUPPOSE TO BE DOING THIS PROCESS) the puppy will even start to eat the food.

My recommendations? File a complaint against the breeders (if you got it from them). It's illegal to give away puppies before 6 weeks!
Hoped I helped. That's how I did it. I helped my aunt cause she breeds German Shepherds and Papillions. :)
